All patients under the age of 18 years

All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.

However, emergent surgical issues arise from time to time.  Delays may occur and appointments may need to be rescheduled. We understand your time is valuable and we appreciate your patience and understanding.

Office Policies

All co-pays, deductibles and other payments are expected on the day of service unless prior arrangements have been made with the billing manager. Co-pays are determined based on the specialist rate as set by your insurance provider. For your convenience we accept VISA, MasterCard and Discover. Checks should be made payable to Dennis L. Fernandez, M.D., P.C.

Our doctors are not in the office every day due to their surgical schedules. Therefore, please be mindful of your prescription refill requirements. Approval of your refill may take up to three business days. Additionally, medication refills will only be addressed during regular office hours (Monday-Friday).

As a courtesy to our patients, we don’t charge a fee to complete FMLA notices, disability paperwork or absence forms; however, we do require five (5) business days to complete any forms requested of our staff members.
